Objective: |
To demonstrate a case of pyocolpos in 23 year old umarried sexually
inactive, regularly menstruating girl with sepsis. | |
Methods: | An unmarried 23 year old female presented with acute pain in abdomen and headache since 2 days. She had recurrent attacks of urinary tract infection for the last 2 months. She had undergone vaginoplasty 8 years ago for cryptomenorrhea. She has ben having regular menstrual cycles with moderate flow. On examination she was having tachycardia and hypotension, with cold peripheral extremities.
Local examination revealed stenosed vaginal introitus with normal external genitalia. Foul smelling purulent discharge was seen coming out thourh the introitus on bimanual per rectal examination. Ultrasound revealed normal size uterus, cystic mass about 60 X 6, 0 X 70 millimeters with mural nodule in pelvic cavity. Under short general anesthesia, pyocolpos was drained. Patient recovered uneventifully.

Results: | After dilation with hegar’s dilator, about 150 milliliter pus was drained out. Cervix was seen clearly, no evidence of collection in uterus. Vaginal packing done for 48 hours. Culture report showed anaerobic organism and Escheria coli growth.

Conclusion: | Possibility of pyocolpos in menstruating women is rate, but it is still a differential diagnosis of acute abdomen in sexually inactive females with a previous history of vaginal surgery.
